Proper accessible structure of a form field within a sentence

Hello,

I need to create a form where a text field is within a sentence and is properly accessible, such as:

This year I will be [_] years old.

The brackets representing a small input[type=text]. Would the text here be the label? DO I need a visually hidden label to explain what's happening?
 
After some thought, I have come up with the following:

HTML:
<label for="retiremonths" class="visuallyhidden">In approximately how many months do you plan to retire?</label>
<p>
  <span class="visual-label" aria-hidden="true">I&rsquo;m thinking about retiring in</span>
  <input type="text" name="retirementmonths" id="retiremonths" />
  <span class="visual-label" aria-hidden="true">months.</span>
</p>

Does this seem acceptable?
