PHP Question about form with Php and Html

Hello. I just learning how to make a Contact form with php and Html. I want to know the following;

When creating a php script, to be included into a form in html ,

This php script should be linked in the head of the html page? Or is it enough to put the PHP script in the form action?

It's enough for it to be in the form action. You don't need to link it elsewhere

as a general good coding practice, your contact.php should consist of PHP db business logic that does input checking and then calls a set of database middleware code like ADOdb (for SQL) or GitHub - sbisbee/sag: A simple but powerful PHP library for talking to CouchDB. (for CouchDB).
and if you need real interactivity (rw access to the db from js), then i can recommend jQuery to achieve that.

Well, to link PHP script with HTML form you can use below code

<form action="process.php" method="post">
    <!-- form fields go here -->
    <input type="submit" value="Submit">
</form>
